Lauren obsessively worries that something bad may happen to her grandfather, that she might say something wrong in front of other people, and that other people around her could be sick and infect her. Lauren copes with these intrusive thoughts through a series of routines, including making breakfast by 9:00 AM to ensure that everything that day is going to be okay. The day that Lauren failed to make breakfast by 9AM, she became consumed with anxiety. Lauren also describes being unable to focus when items are uneven – such as her shoelaces. She must also perform various rituals, such as when in the shower she requires three of the same body washes and the water temperature needs to be exactly the same every day. Any deviation in her routine causes Lauren to stop the shower and wait. Lauren also has to check if the doors are locked at home, and hears that her mom does the same thing.

Arnie describes numerous obsessive thoughts and compulsions that could be considered normal for his line of work as a chef. At work, Arnie checks and rechecks the order of his cooking station at the end of the night. Sometimes, however, Arnie will be driving home when he has to turn around to re-check certain parts of his routine, such as to make sure the stove is turned off. Arnie often worries about food contamination out of concern for the liability with customers at his restaurant and developed a system in order to know where all of his cooking utensils, spices, and other foods will be. Arnie also prefers to be prepared for earthquakes or any other unspecified disaster.

This training title demonstrates Coping Mechanisms and Defenses: Reaction Formation. Reaction formation is acting the opposite way than the unconscious instructs a person to behave, often in an exaggerated and obsessive manner. (Example: If a wife is infatuated with a man other than her husband, reaction formation may cause her to – rather than having an affair – become obsessed with showing her husband signs of love and affection.)

The disorders contained in this grouping can present with varying degrees of severity ranging from mild impairment to incapacitation. Some patients may even have a profound degree of functional impairment that limits their ability to participate in usual activities, potentially affecting every aspect of their lives. Those who experience obsessions and compulsions may find their lives disrupted and controlled by thoughts or behaviors that are entirely unwelcome. While the disorders in this grouping may present with a variety of symptoms, the core symptom set is obsessive thoughts. In some, but not all cases, patients will have repetitive or compulsive behaviors associated with the obsessive thoughts

Ms. Bardin hides one side of her face under her long hair, keeping her left ear hidden. It is painful for her to show her left ear to other people, convinced it is deformed despite doctors telling her that it is perfectly normal in shape, size, and general appearance. She constantly thinks about her ear and how ugly it makes her look. For years has been planning to get surgery to make her left ear normal and symmetrical. This training title highlights the major symptoms of Body Dysmorphic Disorder. Symptoms of Body Dysmorphic Disorder may include: preoccupation with one or more perceived defects or flaws in physical appearance that are not observable or appear slight to others. At some point during the course of the disorder, the individual has performed repetitive behaviors (e.g., mirror checking, excessive grooming, skin picking, reassurance seeking) or mental acts (e.g., comparing his or her appearance with that of others) in response to the appearance concerns. The preoccupation causes significant distress or impairment in social, occupational, or other important areas of functioning. The appearance preoccupation is not better explained by body fat or weight in an individual whose symptoms meet diagnostic criteria for an eating disorder.
