In the past, Natalie had many close relationships but currently feels that she has no one she could consider a friend. Natalie stopped hanging out with her classmates and so they stopped inviting her. Natalie’s social interactions at school, even small ones, are minimal. When she does initiate contact with others, she describes feelings anxious and not wanting to say anything wrong. Though Natalie cannot describe an interaction in which she “said the wrong thing,” she describes seeing the judgment on other people’s face. At home, Natalie has stopped engaging in conversation. Natalie’s mom tries to give her pep talks, encourage her to go outside, and feels that if Natalie just does some things, that she’ll feel better. On the other hand, Natalie’s dad just yells at her, especially when she doesn’t respond. Natalie used to have a good relationship with her father before but since he lost his job and started drinking more she tries to stay away from him as much as possible. Natalie just wants to be alone and for her parents to stop suggesting things.

Brian hasn’t been sleeping well due to his stay in the hospital, with the noise, uncomfortable beds, a whole world going on outside his doors, and thinking often about how many people are dying in the building. When he’s lying alone at night in the hospital room, Brian often thinks about death and that the hospital is a reminder that he can go to sleep and not wake up. Though Brian also thinks about his mortality at home, it happens more intensely when he’s in the hospital and depressed. Brian does not feel rested. Brian’s appetite has also changed due to his stay in the hospital, primarily because the food isn’t good. However, Brian feels his appetite would be fine if food was brought in from outside the hospital. A couple of weeks ago, when Brian’s eating was on his own terms, he had no issues with his appetite.

For example, Brian describes how nurses have missed his veins, injected medication in his muscles, which is extremely painful. Those experiences are enough for Brian not to cause himself more harm. Brian admits to thinking about suicide and weighing whether or not about it’s worth continuing to live, but he’s never formulated a plan to commit suicide. Brian often wonders why he has to have Cystic Fibrosis and exist in this choked off world where things will only get worse.

Recently, Brian checked yes next to a hospital questionnaire that indicates he’s currently having suicidal thoughts and appears almost eager for this interview. Following up on the questionnaire, the nurse asks Brian whether he is suicidal, to which Brian confidentially responds yes. In a speech that is perhaps well rehearsed, Brian expounds that he always likes “to keep suicide as an option on the table.” He explains that suicide is an absolutely viable option since he has Cystic Fibrosis, an incurable disease. Brian details his quality of life, which is miserable but he’s used to it. Recently, Brian has been feeling depressed since he has been stuck in the hospital away from his friends, and contracting pneumonia again, and fears that his lung transplant is failing.

After establishing with Kramer that this conversation is confidential to ensure an open and honest discussion, the interviewing nurse questions Kramer on his experiences with alcohol and other substances. Kramer doesn’t use prescription medication or street drugs, but admits that he used marijuana a couple of times because it made him laugh and have a good time. The nurse observes that Kramer seems down and sad, which Kramer acknowledges is how he’s been feeling over the last few months. Any recent periods of “having a good time” and laughing with friends he attributes to when he smokes. The nurse emphasizes to Kramer that associating these feelings of happiness and having a good time with his use of marijuana is misleading because it’s temporary and in the long run could lead to feeling even worse.

Kramer was brought into the hospital by his mother after threatening to commit suicide following an argument between them. Kramer is quiet and downcast during the interview, depressed with little eye contact and speaks in a low voice. Kramer tells the nurse he has thought about using his grandfather’s gun to kill himself. Upon further questioning Kramer reveals that his friend Martin killed himself in the recent past. Kramer admits that the suicidal thoughts came after the aforementioned argument with his mother who wouldn’t allow him to hang out with his friends anymore because she found out they gave him some marijuana. In her conversation with Kramer, the nurse learns that Kramer is the youngest of five siblings, all of whom are significantly older than him, and his father is currently stationed abroad. Kramer was very close to his grandparents but recently his grandmother passed away and his grandfather’s memory is gone. Kramer now feels he is a burden to his mother and they would be better off without him.

Mrs. Pearson is 75 years old, came into the hospital with her daughter due to sudden gaps in her short-term memory, as she had forgotten to turn off the stove burner and went to take a shower. Mrs. Pearson’s daughter arrived home from work and found the burner on and her mother in the shower. When Mrs. Pearson came out, her daughter told her that it was the second time in 5 days that it has happened. Mrs. Pearson recounts to the interviewer that she was warming up water for a cup of tea and does not recall what happened that the stove remained on. Though Mrs. Pearson’s long-term memory is intact, her recent memory and recall is impaired in the context of grief. Mrs. Pearson’s daughter noticed an impairment in her memory since she learned that one of her cousins passed away last week. During the interview, Mrs. Pearson can recall where she is, her date of birth, town she was born, address of the house she grow up, her both parents’ full names. However, Mrs. Pearson cannot recall the day of the week and day for the month. She remembered the year. The neurological tests are negative for any issues and Mrs. Pearson’s sudden change in cognition can be attributed to depression (from grief), therefore, called pseudodementia.
