This lecture will discuss the treatment of a very difficult group of patients. Those patients presenting class II failing dentition, a downhill dentition that requires high-end implant solutions.
We will look at how to approach the diagnostic stage for these cases. Before we can remove some of the hopeless teeth, we have to think first about the new tooth position which is more aesthetic, not only orally, but also facially, giving a better over-all appearance to the patient.
Function goes hand-in-hand with aesthetics. So, we need to test the new tooth position first before we can finish a case in which we transform a class II situation into a more normalized class I occlusion with nice aesthetics.
We can design and produce a special type of denture with full ceramic teeth and acrylic only on the inside. Because, on the outside, if we remove a thin layer of the acrylic after etching the gingival margins it can be replaced with composite to get a good seal between the ceramic and the composite and optimize the aesthetics.

This lecture presents the biological tissue responses of soft and hard tissues and the effect it has on ideal implant placement and creating esthetic implant restorations. The viewer will learn the ideal 3-dimensional implant position, critical biological factors and the effect they have on long-term peri-implant tissue support. The complex events of wound healing leading to stable epithelium and connective tissue adherence will be shown and the outcome following flap versus flapless techniques discussed. Advances in accurate provisionalization and abutment design optimize the outcome of treatment.

This lecture will outline the concepts in the optimization of esthetic results on implants with a biological approach. The author will stress the need to clinically obtain long-lasting health and stability of the surrounding soft tissues which should be achieved predictably in each and every surgical and restorative implant treatment. The lecture will cover biologically designed abutments, transmucosal biocompatibility of restorative materials, submergence/emergence profiles, connections/disconnections of prosthetic parts, scanning or impression of abutments connected during 1st stage surgery, and explore treatment options from diagnostic models through immediate provisionals to the optimal definitive ceramic-based restorations. The cases will range from single to multiple implants, in complex or simple situations in healed or extraction sites.

Today, dental implants are routinely used to restore missing teeth in all regions of the jaws. However, replacing single, small-sized teeth like upper lateral and lower incisors is often limited due to available mesio-distal space. The diameter of conventional, small platform 2-piece implants is mostly around 3,3 to 3,5 mm. With regard to neighbouring teeth, a minimal distance between implant and root of around 1,2-1.5 mm is recommended in order to maintain interproximal attachment levels at natural abutments. Thus, an overall mesio-distal gap size of at least 6 to 6,5 mm is needed when selecting conventional implants. With the introduction of 1-piece implants, now smaller diameters down to 3 mm are possible without jeopardizing the long-term fracture resistance. In combination with an adequate surgical and prosthetic protocol, these 1-piece implants represent a new treatment option for restoring small sized gaps with a mesio-distal width of down to 5 mm.
