The video first describes how to create a flap and cover the macular hole with and without removal of the ILM from the macula, the latter approach being appropriate when the macular hole is small. Then, the technique in the challenging scenario of macular hole retinal detachment is shown, focusing on two patients who underwent ILM flap surgery with and without retinotomy. In each case, key steps are described in sequence and guidance is provided on the avoidance of pitfalls. The treatment of both has been suggested to decrease the functional decline. Although some forms of hearing loss in the elderly are irreversible, many are reversible. The video aims to familiarize the practitioners with the whole process, thereby increasing confidence of surgical success.There is a significant morbidity and mortality risk on surgical treatment of aneurysms and dissections involving the aortic arch and the descending aorta. For patients with DeBakey type I aortic dissections involving the aortic arch, there has been a focus of debate among cardiovascular surgeons on whether to perform a hemiarch replacement or total arch reconstruction. This video presents a 32-year-old male patient diagnosed with DeBakey type I dissection involving innominate and axillary artery. The surgical plan includes upper hemi-sternotomy and cardiopulmonary bypass set-up, “arch first' reconstruction with a four-branched-graft (Vascutek), aortic root mobilization and sub-annulus suture placement, re-implantation aortic valve-sparing procedure and stented graft implantation into descending aorta and anastomosis. A 28MM four-branched-graft was used for total arch replacement. 'Arch first' reconstruction was performed by sequential anastomosis of the three arch vessels with the branched-graft. Post-operative imaging showed a normal aortic root and arch.

When treating geriatric patients, physicians must keep a differential of systemic diseases that could manifest with ocular symptoms. This is especially crucial when treating elderlies with multiple comorbidities and vasculopathic risk factors. For example, when assessing diplopia patients, one must first attempt to localize the lesion with a thorough motility examination. In addition, clinicians must keep in mind the risk factors such as diabetes, heart disease, hypertension, hypercholesteremia, and compressive lesions. Of these, diabetes is the most common etiology account for 46% of pupils sparing third nerve palsy. Other pathology that must be excluded include giant cell arteritis and myasthenia gravis. Patients must be cared for competency-based aspect of dealing with multidisciplinary clinical problem, and refer patient to appropriate physicians for adequate care. This video covers the unique aspects of all of these things as seen in the aging eye, which physicians across specialties may not have been trained to do.

The video aims to familiarize the practitioners with the whole process, thereby increasing confidence of surgical success.
Open aortic arch surgery and coronary artery bypass grafting (CABG) are considered high-risk operations. Simultaneous operation is also associated with prolonged duration of myocardial ischemia and CPB, thus proper perfusion strategies and myocardial protection contribute to successful outcomes. This video presents a 52-year-old female patient suffering from shortness of breath during exertion for 10 years. She was diagnosed with ascending aorta dilatation without previous surgical treatment. Pre-operative CTA showed DeBakey type I dissection and ascending aorta aneurysm, together with 80% proximal right coronary stenosis. The surgical plan includes cardiopulmonary bypass set up, 'arch first' reconstruction with a four-branched-graft stented graft implantation into descending aorta and anastomosis and re-implantation aortic valve-sparing procedure. Post-operative CTA showed normal aortic root and arch and thrombus formation in false lumen at the level of stent. The patient is discharged uneventfully.

The video describes the surgical strategy for approaching a large mass located in the posterior half of the third ventricle occluding the sylvian aqueduct and causing significant obstructive triventricular hydrocephalus in a 10-year old girl. Hydrocephalus was treated first with an endoscopic third ventriculostomy, and an attempt to biopsy the tumor failed due to hemorrhage from the lesion. It was therefore decided to approach the tumor directly with the aim of the maximum possible safe removal. The three main surgical options are presented: The transcallosal/transforaminal route, the posterior interhemispheric/transtentorial route and the anterior interhemispheric transcallosal/transchoroidal route. The respective advantages and disadvantages of the three approaches are discussed and the reasons for ultimately selecting the transcallosal/transchoroidal route are explained. The rationale for choosing this approach comes from a careful analysis of pre-operative MRI scans on high definition sagittal images, which show how the tumor is located at the entrance of the sylvian aqueduct, whose inlet is extremely dilated, assuming a wide funnel shape. A post-operative MRI shows near total removal of the tumor, which the histology confirmed as pilocytic astrocytoma.
