Dan, teen, currently exhibits signs of moderate cannabis intoxication. Dan tells the nurse that he sometimes drives under the influence of marijuana, and it’s apparent that he’s under the influence with his blood shot eyes and a slur to his words. Dan describes a pattern of cannabis use, often vaping to "level out", or taking a hit from a bong or edibles, and has been doing so since he was thirteen, almost every day. Dan was recently expelled from school, he says primarily because of his grades, but he was also caught selling pot. Dan has held previous jobs but was fired so he now "delivers" pot for a dispensary in exchange for marijuana. Dan insists that he uses pot primarily for anxiety and that when he’s not using he feels stressed out with school, his mom’s expectation, and generally with everything. When Dan is stressed, he can’t sleep. Dan’s mother has become concerned that Dan has been using more pot ever since the family tragedy where Dan’s father committed suicide. Dan admits that he is depressed and using marijuana helps him cope with his loss and get through the day. In addition, Dan is fearful that he may be having visions like his father had, who was diagnosed with schizophrenia.

What they do know is that disrupted or the lack of sleep can be extremely distressing, and in rare cases, even fatal. Most of us have experienced trouble sleeping at one time or another, which may interrupt one or more of these types of sleep. However, for some, trouble sleeping can become habitual, to the point it causes significant distress in an individual's life. These challenges may be due to difficulty falling asleep, intense nightmares, breathing issues, or the effects of a substance or medication. They can even continue into the day, affecting or interrupting an individual's wakeful period. If any of these appear to be the case, the individual may have a sleep disorder, also called a sleep-wake disorder. Because sleep is so vital to our experiences as human beings, individuals with sleep-wake disorders may find these disturbances go well beyond their periods of sleep, negatively affecting every aspect of their lives.

After assurances from the clinician that he should stay and that this conversation is private, Duncan admits that everyone in his class has been posting negative messages about him online. The shaming posts all started with a girl from his study group, Megan, who commented that Duncan should share the answers that he gets from the Tutor Center because of his dyslexia. As a result, Duncan hasn’t slept all weekend, describes pain in his back and a bad headache. From further questioning, we learn that Duncan’s headaches and back pain appear to correlate around his having to take tests. Duncan is extremely frustrated with his schoolwork, unable to what he claims are simple tasks because letters and words all become jumbled in his head and he can’t keep math equations organized. He claims that the teachers at the school don’t care and that he’s seen generally as an idiot. This low self-esteem and poor self-perception has led Duncan to believe he’s not gong to college even though his parents have exceedingly high expectations for him. When asked about his parents, Duncan becomes more upset, saying that they always overreact and that he doesn’t want them to know of his difficulties.
