Practicing emotion-focused therapy (EFT) requires a keen awareness of the client’s body language, breath, subtle microexpressions on the client’s face, and any other nonverbal communication cues expressed in therapy. For this reason, conducting this therapy in an online setting has its challenges. Rooted in a method that integrates strategies from client-centered, Gestalt, and systemic therapies, one important aspect of EFT is recognizing patterns as they are occurring in each moment and from within a highly validating therapeutic relationship. In this video program, Dr. Rhonda N. Goldman demonstrates her approach to therapy remotely with a female client who is suffering from a deep sense of unexpressed grief and unhealed emotional wounds. During the demonstration, Dr. Goldman works with the client to explore underlying emotional and interactional states while simultaneously guiding the client towards a positive transformation by empathetically examining former relationship bonds and accessing healthier emotions. In this video program, Drs. Hadas Wiseman and Orya Tishby demonstrate the application of the core conflictual relationship theme method (CCRT), a therapeutic approach developed by Dr. Lester Luborsky. The core conflictual relationship theme serves as an internal representation of clients’ relationships with early caregivers and guides the therapy in the process of exploring the client’s subjective experiences in current relationships, including the relationship with the therapist.. A dynamic focus of therapy, the CCRT method is used within supportive–expressive therapy to help the client gain awareness of recurring relationship dynamics, or the client’s core conflict. The CCRT model is both a clinical formulation firmly rooted in clinical experience and close observation, as well as a theory-driven method rooted in Freudian psychodynamic theory. During the demonstration session, Drs. Orya Tishby and Hadas Wiseman examine and review the CCRT method from a structured interview—the Relationship Anecdotes Paradigm, or RAP interview, and discuss various interventions demonstrated with a young woman who is coming into therapy to work on relationship issues in her life.
